L'Arte Terapia non migliora i disturbi della Schizofrenia

According to the current study by Crawf ord and colleagues, schizophrenia is a severe mental disorder aff ecting as many as 1 in 100 persons during the lif espan. I n addition to positive symptoms of  hallucinations and delusions, patients also experience negative symptoms of  loss of  energy, impaired attention, and impaired motivation. Although drug therapy is of ten off ered with psychological and social support, art therapy has been discussed as having the potential to reduce symptoms but has rarely been studied as a therapy.
This rater-blinded, parallel-group, randomized controlled trial compares art therapy, activities therapy, and usual care in outpatients diagnosed with schizophrenia.
Art therapy does not improve the mental health or social f unctioning of  patients with schizophrenia — a f inding that is in stark contrast to current United Kingdom national guidelines that recommend that all such patients be ref erred f or some type of  art therapy.
A 3-arm, rater-blinded, randomized controlled trial showed no diff erences in global f unctioning and mental health symptoms between patients receiving weekly group art therapy plus standard care, weekly activity groups plus standard care, or standard care alone.
"These f indings challenge current national treatment guidelines that clinicians should consider ref erring all people with schizophrenia f or arts therapies," the researchers, led by Mike Crawf ord, MD, prof essor in mental health research, I mperial College London, United Kingdom, write.
The study is published online February 28 in BMJ.
According to the investigators, art therapy is of ten used as an adjunctive treatment f or schizophrenia, particularly to address the so-called "negative" symptoms of  the disease, which can include loss of  energy and motivation and impaired attention. However, they add, f ew studies have assessed its impact on clinical outcomes.
The Multicenter Study of  Art Therapy in Schizophrenia: Systematic Evaluation

 recruited 417 individuals aged
18 or older who had been diagnosed with schizophrenia.
Participants were randomly assigned to receive 12 months of  weekly group art therapy plus standard care

.
Art therapy patients were given access to a range of  art materials and were encouraged to use these to express
themselves f reely. Participants in the activity group were encouraged to take part in activities such as playing board games,
watching and discussing DVDs, and visiting local caf es.
The study's primary outcome measures included global f unctioning, assessed using the global assessment of  f unctioning
scale, and mental health symptoms, assessed using the positive and negative syndrome scale. The assessments took
place 24 months af ter randomization.
The investigators f ound no diff erences in global f unctioning and mental health symptoms among the 3 study groups. No
diff erences in social f unctioning and satisf action with care were f ound between art therapy and standard care groups.
During the 2-year f ollow-up period, the number of  total symptoms of  schizophrenia decreased. However, the investigators
f ound no diff erences in primary outcomes among the 3 trial arms.
The adjusted mean diff erence between those assigned to art therapy and those assigned to standard care alone on the
global assessment of  f unctioning scale was -0.9
(
95% conf idence interval [CI ], -3.8 to 2.1; P = 0.57
)
. On the positive and
negative syndrome scale, the adjusted mean diff erence was 0.7
(
CI , -3.1 to 4.6; P = 0.71
)
.
The adjusted mean diff erence between those assigned to art therapy and those assigned to activity groups on the global
assessment of  f unctioning scale was -1.1
(
CI , -4.0 to 1.8; P = 0.47
)
. On the positive and negative syndrome scale, the
adjusted mean diff erence was 3.1
(
CI , -0.7 to 6.9; P = 0.11
)
.
Similarly, secondary outcomes did not diff er between the art therapy group and the standard care groups.
